    • Collate all the details of your accident you have available, including:

      1. the time and date of the accident

      2. what happened

      3. the name and address anyone injured & any witnesses

      4. any photos & video/ surveillance footage

    • Visit your Doctor and request a First Certificate of Capacity (Workers’ Compensation Medical Certificate) which confirms your injury and outlines your work capacity and work restrictions

    • Submit your Claim Form together with a copy of your First Certificate of Capacity to your employer as soon as possible

    • Your employer will then forward the documents to their insurer for assessment

    • It is recommended to submit your claim form to your employer within 12 Months of your accident injuries occurring
