Visit your Doctor and request a First Certificate of Capacity (Workers’ Compensation Medical Certificate) which confirms your injury and outlines your work capacity and work restrictions

Download a copy of the Workers’ Compensation Claim Form available via the WorkCover WA Website at:
Complete the form carefully making sure you include a description of all your injuries
Submit your Claim Form together with a copy of your First Certificate of Capacity to your employer as soon as possible
Your employer will then forward the documents to their insurer for assessment
It is recommended to submit your claim form to your employer within 12 Months of your accident injuries occurring
